Day 2
The Application for Benefits is filed electronically and Notice is sent to the IW’s employer.
The IW is seen by the Advance Practice Nurse (APN) at her primary doctor’s office for the first time post injury the day after our initial consultation.
The APN excuses her off work from date of injury through 2 weeks from initial appointment.
An off work note is generated and e-mailed to me.
Thanks to My Chart the IW is able to access the office note from first visit with the APN. The office note is also scanned and e-mailed to me.
The office note is reviewed. A history of the work injury is noted.
The IW is noted to have abdominal pain. The IW is also noted to be nervous and anxious.
Imaging obtained at the ER of the abdomen is reviewed.
Preliminary Diagnosis includes abdominal pain following workplace assault, Generalized Anxiety Disorder (worsened with recent event) and Depression single episode.
Treatment plan includes follow up in two weeks.
